Our services are designed to empower individuals to live, work, learn, and contribute in ways that are meaningful to them. Through personalized support, we help people build skills, access opportunities, and create the lives they want for themselves.

  • Services Provided

  • These services are designed to provide one to one individualized supports. All services are determined using a person centered planning process and follow the goals and choice of the individual served.

    Services can include:

    • Job Search Planning  (Discovery)

    • Job Development and Negotiation  

    • Post-Employment Support 

    • Post-Secondary Education Supports 

    • Micro-Enterprise Development and Supports

    • Volunteer Development and Support

  • Adaptive Skills Training helps individuals develop the practical skills needed to live, work, and participate in their communities. Skill-building takes place in real-life settings and through everyday experiences, allowing individuals to learn, practice, and apply new skills in meaningful ways.

    Skill building may include:

    • Communication

    • Social Functionning

    • Safety Awareness

    • Personal Care

    • Use of Technology

    • Self-Support Strategies

    • Decision Making and Advocacy

    • Other related areas that are critical for community inclusion

Self-Determination Program Services

Individuals who are participants in the Self-Determination Program have a variety of service options within our agency. All services provided reflect our philosophy of supporting individuals to have the most positive control over the lives they choose to lead for themselves.

  • Independent Facilitation Services are designed to support you in directing and managing the services and supports you choose through the Self-Determination Program. As your Independent Facilitator, we help you turn your Person-Centered Plan into action by assisting with recruiting, hiring, and training staff, as well as supporting you in identifying and working with vendors and other service providers.

    We also help you connect with resources in your community that align with your goals and support your vision for a meaningful and self-directed life. In addition, we provide ongoing support to review your monthly spending to help you stay within your individual budget and make informed decisions along the way.

    Our role is to provide guidance, coordination, and practical support so you can make informed choices and maintain greater control over your services and daily life.

    Independent Facilitation Services are billed at $100/hour (in 15-minute increments) and are funded through your Individual Budget. Services can be provided virtually or in person, depending on your needs, preferences, and location.

  • Person-Centered Planning is a process that helps you identify what matters most to you, clarify your goals, and create a vision for your future. This is your plan, and you guide the process with your voice, choices, strengths, and aspirations at the center.

    Our role is to support you through each step. We take time to get to know you, help you identify your circle of support, and bring together the people who know and care about you to contribute to your planning process.

    We help to coordinate and facilitate planning meetings, support meaningful conversations about what’s working and not working in your life, and help you explore your goals and the supports you may need to reach them. After these meetings, we develop a written Person-Centered Plan that reflects your priorities and outlines clear goals, action steps, timelines, and the people involved in supporting you.

    The final plan serves as a practical roadmap to guide your next steps in the Self-Determination Program.

    Your Initial Person-Centered Plan is funded through your Regional Center, up to $1,000. We are therefore able to provide 10 hours of planning services based on our rate of $100/hour.

  • We offer direct services and supports designed to help you achieve the goals outlined in your Person-Centered Plan and Individual Program Plan. These services are fully individualized based on your goals, interests, and support needs.

    Supports may include, but are not limited to:

    • Discovery and Customized Employment Development and Support

    • Community and Civic Involvement

    • Self-Advocacy

    • Post-Secondary Education Support

    • Micro-Enterprise Development and Support

    • Benefits Counseling

    Each service is tailored to the individual, ensuring supports are responsive to your unique needs and preferences. Rates vary depending on the type of service requested. Please contact us directly to discuss the support you are looking for, and we will provide current hourly rate information.

Benefits Planning

Our Benefits Planning Services help individuals who receive Social Security understand their benefits, the impact of earned income, and strategies for building long-term financial stability. Through personalized counseling, we help people make informed decisions about employment, education, self-employment, and other life goals. Services are available through private pay, Self-Determination Program (SDP) funding, and/or the Ticket to Work program.

  • Services available through Ticket to Work include:

    • Individualized analysis and advisement regarding Social Security benefits (SSI, SSDI, Child Disability Benefit)

    • Development of PASS plans

    • Assistance in utilizing work incentives

    • Advisement regarding earnings and their impact on social welfare programs including but not limited to

      • Medi-Cal

      • Housing assistance

      • CalFRESH

    • Resume development support

    • Financial wellness advisement

  • Benefits Counseling helps individuals who receive Social Security benefits understand how employment, self-employment, education, and other life changes may affect their benefits. Through personalized guidance, we help people make informed decisions, maximize available work incentives, and plan for greater financial stability and independence. Our goal is to ensure that individuals have accurate information and the confidence to pursue their goals while understanding their benefits and available options.

    These services are available via private pay or through the Self-Determination Program.
